Registered Nurse | Anticoagulation Clinic

Avera Health is seeking a compassionate and detail‑oriented Registered Nurse to join our Anticoagulation Clinic within our Healthcare & Medical Services department. Based in our collaborative, patient‑focused environment, this RN plays a key role in managing the care of patients receiving anticoagulation therapy such as warfarin and direct oral anticoagulants. The nurse will perform comprehensive patient assessments, review lab results (including INR and other coagulation studies), adjust dosing under established protocols or provider direction, and provide thorough patient education on medication adherence, dietary considerations, bleeding risk, and safety. This role involves close coordination with physicians, pharmacists, and other members of the care team to optimize therapeutic outcomes, prevent complications, and ensure continuity of care across settings. The Registered Nurse will conduct in‑person and telephone or telehealth follow‑up visits, triage patient concerns related to anticoagulation, and respond promptly to urgent clinical changes using sound nursing judgment. Accurate documentation in the electronic health record, adherence to evidence‑based guidelines, and participation in quality improvement initiatives are essential components of the position. The RN will contribute to clinic workflow efficiency by prioritizing patient needs, managing caseloads, and supporting standardized care pathways while maintaining a compassionate, respectful approach to every interaction.
