Continuing Care Coordinator- Outpatient Program

Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Continuing Care Coordinator will provide rehabilitation services to patients as a part of their overall treatment. The Continuing Care Coordinator will work on a Full-Time basis with our Meadows CCBHC Open Access Program.
The schedule will be Monday through Friday, 7:30am-4:00pm.
In addition to Helio Health's comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!
Pay Rate: $20.00 - $25.00 per hour.
Responsibilities- Coordination of continued treatment and/or care of patients leaving services with internal and external program and/or resources.
- Development of aftercare planning in collaboration with the patient and any significant other(s) the patient chooses to involve.
- Assisting with the physical discharge process.
- Explaining and presenting resources within the community that will aid in the patients recovery process once they leave the program.
- Coordination of care with referral sources.
- Case management to include assessment, screening, orientation, referral, counseling, crisis response, patient education, reportinq and record keepinq and consultation with other professionals.
- Guiding patients through aftercare planning to include but not limited to setting up aftercare appointments.
- Other duties as assigned by supervisor
- Represents agency to community
- Participates in program, administrative and clinical committees as assigned.
- Associates degree required/ Bachelor's degree preferred.
- One year employment experience in the Human Services field preferred.
- Equivalent alternative combination of education and experience may be substituted for the educational requirements.
- Qualified Health Professional (Q.H.P) preferred.
- Must demonstrate steady progress towards Qualified Health Professional status if not currently Q.H.P.
- Knowledge of addiction/recovery, behavioral health, self-help group process and principles of counseling.
- Proficiency with basic computer applications.

Salary and hourly compensation ranges are provided in accordance with NYS law and are based on Helio Health's good faith belief of what is accurate at the time of posting. Salary and hourly compensation offers are based on candidate's education level and experience relevant to the position and also take into account information provided by the hiring manager and program.
