Job SummaryThe Psychiatric Mental Health Nurse Practitioner (PMHNP) will be responsible for the on-site delivery of psychiatric services to young individuals enrolled in various programs at Hanna Center, including individuals age 12-25 participating in the Coordinated Specialty Care program who meet criteria for Clinical High Risk for Psychosis (CHRp) or First Episode of Psychosis (FEP) and individuals 15-20 residing on campus as part of the Transition Housing Program (THP) and Non Public School (NPS). This role involves close collaboration with the supervising psychiatric medical director, therapists, employment and education specialists, peer support specialists, family support specialists, and residential youth counselors.
Essential FunctionsConduct psychiatric evaluations and ongoing assessments of CHRp, FEP, and residential patients.
- Review outside medical records, perform diagnostic assessments including taking medical histories and performing physical exams, determine appropriate medical work ups including labs/imaging, communicate recommendations for diagnostic assessments to outside medical providers, interpret medical work up results, refer to outside medical specialists if appropriate, prescribe medication if appropriate, and monitor progress and treatment efficacy.
- Conduct medication management, including routine follow-up appointments, post-medication change visits, and higher-acuity check-ins as needed.
- Coordinate lab orders and follow-up care in collaboration with primary care providers and other medical professionals.
- Perform crisis interventions, including but not limited to safety assessments and determination of need for involuntary hospitalization.
- Administer injectable psychotropic medications.
- Establish and maintain therapeutic rapport with patients and productive working relationships with involved support persons.
- Maintain caseload of up to 40+ patients per .4FTE
- Conduct frequent appointments as deemed appropriate to meet individual patient needs.
- Provide 1:1 psychiatric follow-up for residential clients as clinically indicated.
- Maintain clinical records in accordance with mandated standards.
- Perform chart reviews, complete documentation, and manage prescription refill orders.
- Collaborate with a multidisciplinary team that includes the psychiatric medical director, therapists, case manager, employment and education specialist, peer support specialist, and family support specialist.
- Engage in care coordination and case management with therapists, caregivers, social workers, and other members of the multidisciplinary team.
- Participate in case consultations with direct care staff and treatment team members to support integrated care planning.
- Attend and participate in weekly staff meetings.
- Attend and participate in clinical supervision with supervising psychiatrist.
- Consult and provide ongoing education to non-medical staff in psychopharmacology and medical aspects of patient care.
- Attend and participate in all assigned trainings for professional, program, and agency development.
- Comply with all federal regulations for the sake of privacy and security of patient information.
- Maintain quality assurance standards, adherence to MediCal/MediCare documentation requirements, and data collection for measured outcomes.
- Be available to program staff for urgent questions outside of scheduled work hours by maintaining flexibility to respond to urgent clinical questions, including medication side effects and emergency refill needs.
- Additional duties, as needed, within the scope of practice.
Supervisory ResponsibilitiesThis position has no direct reports.

- Maintain accurate, timely documentation in adherence with MediCal/MediCare and HIPAA standards.
- Utilizes electronic health records and digital communication tools efficiently.
- Actively participates in training, supervision, and ongoing learning to maintain clinical excellence.
Education and Experience- Licensed to practice as a Psychiatric Mental Health Nurse Practitioner in the state of California.
- Holds an active Furnishing Number and DEA registration number for controlled substances (Schedule II-V)
- 5+ years experience as a PMHNP or 2+ years experience as a PMHNP working with early psychosis patients preferred.
- Proficiency in psychiatric practice, including but not limited to patient safety best practices, psychopharmacology, general medical knowledge, and respect for patient autonomy and privacy.

- Basic Life Support certification.
- Bilingual in Spanish preferred.
- Effective communication skills to receive and disseminate information clearly and professionally.
- Knowledge of basic technology, including Outlook and electronic health record systems.
Additional Eligibility RequirementsMust clear background check for a Community Care Licensed facility, including physical, TB, and drug test.
Physical RequirementsSit for extended periods of time and the ability to manage repetitive hand/wrist movements while using a computer keyboard and/or the phone. Bend, squat, stoop, and walk- up a flight of stairs.
Lift and/or carry up to 20 lbs., occasionally. Duties are primarily performed in an office environment at a desk or computer terminal.
Work EnvironmentPosition is in person on site at Hanna Center. Typical office and clinical environment. Responsibilities may include offsite meetings, trainings, or other activities as required by business needs.
