Registered Nurse (RN) – Home Health

Location: Monongalia, Preston, Tucker, Grant, and Wetzel Counties, WV
Position Type: Contract
Work Location: Onsite / Field-Based
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
On-Call Requirement: Minimum of one weekend per month
Mileage Reimbursement: $0.54 per mile
Productivity Requirement: 30 points per week

Position Overview

We are seeking an experienced and compassionate Registered Nurse (RN) to join our Home Health team. The ideal candidate will have a strong background in home health nursing, proficiency with Homecare Homebase (HCHB), and extensive experience completing OASIS assessments. This role is responsible for delivering high-quality patient care, coordinating services across disciplines, and ensuring compliance with all clinical, regulatory, and documentation requirements.

The Registered Nurse will assess patient needs, develop and implement individualized care plans, provide case management services, and support health maintenance and disease prevention efforts in the home setting.

Required Qualifications

• Active West Virginia Registered Nurse (RN) License
• Previous Home Health Nursing experience required
• Experience with OASIS assessments and documentation
• Proficiency with Homecare Homebase (HCHB)
• Strong clinical assessment and case management skills
• Excellent communication and interdisciplinary collaboration abilities
• Valid driver's license and reliable transportation

Essential Responsibilities

Clinical Care & Patient Assessment

• Conduct comprehensive patient assessments and ongoing evaluations in the home setting.
• Develop, implement, and monitor individualized care plans.
• Observe, assess, plan, implement, and evaluate nursing interventions for patients with acute, chronic, and complex healthcare needs.
• Monitor patient progress and communicate condition changes to physicians and other healthcare providers.

Case Management & Care Coordination

• Coordinate care among physicians, therapists, caregivers, and other healthcare professionals.
• Collaborate with interdisciplinary teams to address patient health, safety, and wellness needs.
• Maintain communication with inpatient nursing teams and discharge coordinators when patients are hospitalized.
• Ensure timely scheduling and follow-up of medical appointments and treatment plans.

Medication Management

• Review, monitor, and manage medication regimens as prescribed by physicians.
• Ensure compliance with medication administration, storage, access, error reporting, and disposal policies.
• Coordinate medication-related communications with providers and caregivers.

Documentation & Compliance

• Complete and maintain accurate OASIS documentation, nursing notes, care plans, and required clinical records.
• Complete daily, monthly, quarterly, and annual nursing documentation as required.
• Transcribe physician orders and ensure timely processing of medically related reports.
• Maintain compliance with Medicare, state, agency, and accreditation standards.

Leadership & Education

• Provide clinical oversight and direction to Licensed Practical Nurses (LPNs) and other support staff as needed.
• Conduct new employee orientation and annual health and safety training.
• Educate patients, caregivers, and staff on disease prevention, health maintenance, and person-specific care needs.
• Promote infection prevention practices and ensure proper maintenance of equipment and supplies.

• Home Health case management experience
• Knowledge of Medicare Home Health regulations
• Excellent organizational and time management skills
• Ability to work independently in the field while maintaining strong communication with the care team
