Total Care Connect (TCC) is a mobile integrated health organization delivering in-home clinical and preventive care to members across Ohio and surrounding regions. As a tech-enabled, field-based care delivery organization, our teams provide a range of services including preventive care, chronic condition support, transition-of-care visits, member engagement, and navigation. Compensation & Benefits

• Hourly pay: $28 - 30/hr based on experience

• Benefits package for full-time employees: health, dental, vision insurance; paid time off; 401(k)/retirement; disability.

Position Summary

TheCommunityParamedicdelivershigh-qualityin-homeclinicalcareaspartofTotalCareConnect’sMobile Integrated Healthcare programs. This role focuses on proactive patient care for high-risk populations, supporting hospital readmission reduction, emergency department avoidance, and improved chronic disease management. Community Paramedics conduct scheduled patient visits, perform clinical assessments, deliver care within established protocols, and collaborate with virtual providers and interdisciplinary care teams to provide coordinated, patient-centered care.

Community-Based Clinical Care

• Conduct scheduled patient visits within community-based careprograms.

• Performcomprehensivepatientassessmentsincludingvitalsigns,symptomevaluation,medication reconciliation, and environmental safety review.

• Deliver care within established clinical protocols and standingorders.

• Providetreatmentssuchaswoundcare,medicationadministration,andpoint-of-caretestingwhenapplicable.

• Identifyearlysignsofclinicaldeteriorationandescalateconcernsthroughclinicaloroperationalpathways.

Care Coordination & Patient Support

• Support continuity of care following hospital discharge or acute medicalevents.

• Assistpatients in understanding care plans and follow-upappointments.

• Identify barriers torecoveryincluding environmental or social determinants ofhealth.

• Collaboratewithcarecoordinationteamstosupportpatientstabilizationandlong-termhealthmanagement.

Documentation & Communication

• Completeaccurateandtimelydocumentation within the electronic patient care recordsystem.

• Ensure documentation supports clinical accuracy, medical necessity, and payerrequirements.

• Maintain communication with operational teamsregardingvisit updates and patientneeds.

• Participate in documentation reviews and quality improvementinitiatives.

Required Qualifications

• Current Ohio Paramedic certification in goodstanding

• Minimum 2–3 years of paramedic fieldexperience

• Valid driver’s license with acceptable drivingrecord

• Strong clinical assessment and patient educationskills

• Experience using electronic documentationsystems

Work Environment

• Field-based role providing care in patient homes, partner offices, or community clinicalsettings.

• Travelbetweenpatienthomes,partneroffices,andoperationallocationswithintheassignedregion

• Exposuretovariedhomeenvironmentsincludingpets,limitedlighting,clutter,stairs,and environmental hazards.
