Excellent Home Care Services is hiring a Therapist / Home Care Clinical Supervisor (OT or SLP) for our Brooklyn location to manage a team of Field Therapists. The ideal candidate will possess extensive clinical expertise in therapy, with a focus on patient rehabilitation, care plan development, and interdisciplinary collaboration.
Duties
- Strict adherence to applicable NYS and federal regulations
- Overseeing the development and implementation of individualized treatment plans for patients.
- Coordinate discharge planning and collaborate with healthcare professionals to ensure seamless patient transitions between care settings.
- Serving as the primary point of contact between the Agency and your Therapy Team
- Familiarity with clinical documentation standards, EMR systems (Electronic Medical Records), and outpatient clinic operations.
- Excellent client communication skills and the ability to collaborate effectively with multidisciplinary healthcare teams.
Requirements include:
- Valid and current NYS Therapy License in your specialty
- 1 year of clinical experience
- Certified Home Health Agency (CHHA) experience preferred
- Strong attention to detail
- Excellent oral and written communication skills
- Ability to multitask, meet deadlines, and problem-solve
- Working knowledge of Medicare/Medicaid and other Insurance programs preferred
