Wound Care Coordinator/Restorative Nurse

Job Summary Location: New Rochelle, New York • Grant Funded: No • Department: SEC Nursing Administration • Work Shift: Day • Work Days: MON-FRI • Scheduled Hours: 8:30AM-5PM • Scheduled Daily Hours: 7.5HOURS • Pay Range: $96,000.00-$120,000.00 Job Summary The Wound Care Coordinator works in conjunction with the Director of Nursing Services. In this position, he/she supervises the areas of Nursing Services which are related to wound care and restorative programs, resident abuse investigations, staff performance and interface with other departments such as rehab, dietary, social services, housekeeping and managed care organization. Responsibilities Assess residents to determine their risk factors for contractures, skin breakdown and deterioration in ADLs in order to develop a nursing rehab plan of care. Monitor the effectiveness of the care plan through direct observations of care rendered through weekly wound rounds assessment. Develop care plans for each resident with a current pressure sore, or evaluated as high risk for the development of pressure sores. Active role in identifying and addressing quality issues pertaining to the development, prevention of wounds and implementing corrective action plans as necessary. Qualifications Bachelor’s Degree (required). Valid NYS RN licensure. Previous experience in a similar role in a long‑term care facility. Wound and Infection Control knowledge. MDS knowledge. Care planning and Assessment Experience.
