Physical Therapist Assistant (PTA) – Full Time Location: Sauk City, WI Greenfield Rehabilitation Agency – Where Passion Meets Purpose
Greenfield Rehabilitation Agency is excited to announce an opportunity for a Physical Therapist Assistant (PTA) to join our therapy team in Sauk City, WI.

Your Responsibilities
- Provide compassionate and effective physical therapy treatments under PT supervision
- Implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Support residents in achieving personal goals and rehabilitation milestones
- Accurately document care and maintain compliance standards (experience with Net Health a plus)
- Collaborate with therapy and nursing teams to ensure patient-centered outcomes
- Educate residents and families on safe techniques and strategies to maximize independence
- Participate in team meetings and contribute to care planning

Qualifications
- Graduate of an accredited school with an associate degree as a physical therapist assistant.
- Certification or license as required by the State of Wisconsin to practice as a Physical Therapist Assistant.
