Caring Minds Health Services LLC is seeking individuals and families in Arizona interested in becoming Adult Developmental Home (ADH) or Child Developmental Home (CDH) providers.
As an ADH/CDH provider, you will welcome an individual with developmental disabilities into your home and provide daily care, supervision, and support in a safe, supportive, family-centered environment.
Requirements
Applicants must:
Be at least 21 years old.
True passion to care for others
Have suitable living space in their home.
Have at least one additional adult, age 18 or older, living in the home who is willing to participate in the licensing process and be added to the license application.
Complete required training, background checks, and home licensing requirements.
Be willing to provide a safe, caring, and supportive home environment.
What We Offer
Caring Minds Health Services LLC provides guidance, training, and ongoing support to help qualified applicants successfully become ADH/CDH providers.
Licensing Guidance: Step-by-step assistance throughout the application and home licensing process.
Training Support: Guidance in completing required training and certifications to meet licensing requirements.
Application Assistance: Help with required paperwork, documentation, background checks, and preparing for home inspections.
Ongoing Support: Continued assistance and guidance as you begin providing care in your home.
Our goal is to make the licensing process as smooth as possible and help you feel confident and prepared to welcome an individual into your home.
