Travel RN Case Management - Neuro Step Down
Location: Fort Worth, TX
Assignment Duration: This is a 13-week contract position.
Schedule: 8:00 AM - 4:30 PM
Job Description: We are seeking a dedicated Travel RN Case Manager to join our team in the Neuro Step Down unit. The successful candidate will be responsible for providing comprehensive case management to patients, ensuring their needs are met through effective communication and collaboration with a multidisciplinary team.
Responsibilities:
- Manage patient care throughout the healthcare continuum.
- Collaborate with healthcare providers to create and implement patient care plans.
- Conduct assessments to determine patient needs and resources.
- Educate patients and families on care options and resources available.
- Maintain accurate documentation in EMR systems (Meditech, Careport).
Required Experience / Certifications / Licensure:
- Must have at least 1 year of Acute Care Case Management experience.
- Experience with Meditech and Careport is preferred.
- A valid RN license is required.
