RN Home Health Admission Nurse Patient Intake & Care Coordination

Masonicare Corp in Wallingford, CT is seeking an Admission Nurse RN to ensure safe and quality delivery of home health services. You will admit patients, reconcile medications, and complete Oasis data while coordinating care with the nurse case manager.
Primary duties include coordinating the plan of care, maintaining continuity of client care, and supporting the agency’s quality improvement efforts through accurate documentation and timely communication.
