Description

• Assess patients in both inpatient and outpatient settings and craft tailored treatment plans
• Deliver physical therapy interventions focused on mobility, strength, balance, and functionality
• Work closely with nurses, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes
• Empower patients and families with knowledge about therapeutic goals, discharge processes, and home exercise plans
• Maintain thorough documentation of evaluations, treatments, and patient progress per facility and regulatory guidelines
• Engage in care team discussions and interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (such as neurologic or geriatric) are advantageous, but not mandatory

Skills

• Prior experience is strongly preferred
