Remote RN Care Manager - Transitional Care Leader

Loma Linda University Health is seeking a Care Manager-Registered Nurse (RN) for Transitional Care Management. This role supports patients to prevent hospitalizations, coordinating care across providers and services with expert assessment and planning. Remote scheduling is available up to two days per week after successful completion of a 90-day probationary period.
