Case Manager

Rate $23-$25 an Hour The Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program (THP) provides direct support and intensive case management services to assist Transitional Aged Youth (TAY) ages 16 to 21 years, with meeting individualized goals while connecting them with resources in the community. Average case load is 12 youth. The position is collaborative and coordinates care on an interdisciplinary team. QUALIFICATIONS REQUIRED Bachelor’s Degree in one of the following areas: Marriage, Family and Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Clinical Psychology, Educational Psychology, Education with an emphasis on Counseling, Social Work or Social Welfare, an area that includes counseling or psychotherapy content required for licensure as a Licensed Professional Clinical Counselor. Bachelor’s Degree in one of the following areas: Human Services or Behavioral Science, AND at least two (2) years of experience in a public or private child welfare social services setting, with specific experience working with TAY 16 – 24 years of age. CCL approval as an Excepted Case Manager prior to start date. Experience working with the TAY population. Driver’s License. #J-18808-Ljbffr
