Job Description

Deliver high-quality patient-centered care as a Home Health Registered Nurse, providing skilled nursing services to individuals in the comfort of their homes. This field-based contract assignment offers engaging, autonomous work and the opportunity to make a significant difference in patient outcomes while collaborating with an interdisciplinary care team.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes within assigned territory (mileage reimbursement provided)
• Caseload: 2532 points per week (approximately 35 patients per day)

Key Responsibilities:

• Conduct comprehensive patient assessments using OASIS and develop individualized care plans.
• Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease oversight.
• Monitor progress, recognize changes in patient condition, and communicate promptly with care teams and physicians.
• Provide patient and family education on disease management and preventive care.
• Accurately document all care activities in Home Care Home Base (HCHB) system.
• Collaborate closely with therapists, medical social workers, and home health aides for coordinated care delivery.
• Participate in case conferences, staff meetings, and ongoing training.

Required Qualifications:

• Active, unencumbered RN license in the state.
• At least 1 year of recent Home Health nursing experience; field experience strongly preferred.
• Proficiency in HCHB documentation and OASIS assessment.
• Solid clinical assessment, critical thinking, and communication skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently, organize schedules efficiently, and provide compassionate care.

$2,016 - $2,132/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
