CVS Health is seeking a licensed Behavioral Health Case Manager to support members with complex needs from a fully remote home office. You will conduct assessments, develop care plans, coordinate with providers, and assist with crisis intervention and follow-up from a telephonic care team. Ideal candidates hold an active clinical license, have 3+ years of post-master's experience in behavioral health and case management, and can thrive in a fast-paced, remote work environment with strong #J-18808-Ljbffr Capacity Path
