Travel RN – CVICU (Cardiovascular ICU)

⏰ Shift & Weekly Hours

• Shift Type: 12-hour shifts (typical ICU scheduling)
• Schedule: Self-scheduling + block scheduling allowed (based on unit needs)
• Weekend Requirement: Every other weekend
• Holiday Requirement: As needed based on unit staffing
• Call Requirement: No call / standby required
• Total Weekly Hours: 36 hours/week (3 × 12-hour shifts)

Unit Overview

• Bed Capacity: 18 beds
• Acuity Mix:
  • ICU status patients
  • Step-down status patients
  • Fresh post-op cardiac patients
• Unit Type: Cardiovascular ICU (CVICU)
• Population: High-acuity cardiac surgical + critical care patients

⚖️ Nurse:Patient Ratios

• 1:1 – Fresh open hearts / devices
• 1:2 – ICU status patients
• 1:3 – Step-down status patients

Required Experience (Must Have – Strict)

Candidates must have ALL of the following:

• Fresh post-op open heart experience (>6 hours post-op minimum required )
• CRRT (Continuous Renal Replacement Therapy)
• Temporary pacemakers + permanent pacer care
• Acute MI experience (pre/post cath lab)
• Mediastinal & chest tubes
• Mechanical ventilation (vent/trach vent)
• Hypothermia protocol (post cardiac arrest care)
• Arterial line management
• Experience with multiple ICU drips (listed below)

⭐ Preferred (Strong Advantage)

• IABP (Intra-Aortic Balloon Pump)
• Impella device experience
• Swan-Ganz / Hemodynamic monitoring

Common Diagnoses / Patient Types

• STEMI / NSTEMI
• Atrial fibrillation & heart blocks
• Bradycardia / tachyarrhythmias
• Cardiac arrest
• Cardiogenic shock
• Hypotension / hypertension crises
• Heart failure
• Post cardiac surgery (CABG, valve repair/replacement)
• Thoracic surgery (VATS, lobectomy, etc.)
• Pulmonary embolism

• Cardizem
• Cordarone (Amiodarone)
• Cardene
• Nitroglycerin
• Esmolol
• Cleviprex
• Milrinone
• Dopamine / Dobutamine
• Epinephrine / Levophed / Vasopressin
• Insulin / Heparin / Aggrastat
• Lasix / Bumex
• Sedation: Precedex, Propofol, Versed, Fentanyl

Clinical Requirements

• BLS – Required
• ACLS – Required
• Must be able to manage complex ICU-level cardiac patients independently

‍⚕️ Staffing & Support

• PCT Support: 1–2 per shift
• Hospitalist/Intensivist: In-house
• RT Coverage: 24/7 in ICU
• Monitor Techs: Remote monitoring system
  • RN reads own strips every 4 hours
  • Philips monitors used

Equipment Managed in Unit

• Mechanical ventilators
• Trach ventilators
• BiPAP / CPAP
• High-flow oxygen
• Trilogy ventilators
• Advanced cardiac devices (IABP, Impella if applicable)

Floating Policy

• Travelers float first
• Primarily float to:
  • ACU
  • ICU
• May float to Med-Surg / Tele
  • Max ratio: 1:6 patients

Scrub / Dress Code

• Color: Royal Blue scrubs only
• Must comply strictly with dress code

Scheduling

• Self-scheduling allowed
• Block scheduling often approved
• Subject to unit needs during schedule balancing
• Flexible but priority is staffing coverage

Orientation

• 1 week total orientation:
  • EPIC training
  • Online modules
  • Unit-based precepted orientation
