Skilled Nursing Facility Physician Opportunity: Full‑time opening for a Geriatrician/Skilled Nursing Care Physician. We are seeking a Geriatrician or IM/FM trained Physician to cover skilled patients and follow long‑term care patients in Napa, CA. This position is essential to cover our Skilled Nursing Facility patients in Napa, which spans four facilities for both skilled and custodial patients, as well as home‑based palliative care patients. Geriatrics and/or Hospital Medicine experience is preferred. The physician will travel to/from Skilled Nursing Facilities and a Napa medical office. We are looking for a physician with a passion for improving the quality of life of our most vulnerable population and their families in the post‑acute and long‑term care arena. Responsibilities Cover skilled patients in the facilities. Follow long‑term care patients across the four facilities. Travel to and from Skilled Nursing Facilities in Napa and the Napa Medical Office. Provide care in the post‑acute and long‑term care setting. Qualifications Geriatrician or IM/FM trained Physician. Experience in geriatrics and/or hospital medicine preferred. Full‑time commitment. Ability to travel within the Napa region. Strong commitment to patient care and quality improvement.
