RN Case Manager

RN Case Manager-ALP

Located in Brooklyn, NY

*Salary: $100K to $105K per year (dependind on experience)*

Responsibilites:

Will take responsibility for resident care, supervision of staff, working closely with MD and picking up MD orders, writing case notes and ensuring all residents have their 6 and 12 month medical evaluations. Act as the on-site point of contact for operational issues when administration is unavailable.


Qualifications:

• Must have a valid New York RN license
• UAS certification and experience completing UAS assessments strongly preferred
• Must have prior experience as a Case Manager
• Must have experience in a geriatric setting, preferably an assisted living program
• Strong written and verbal communication skills
