RN Case Manager - ECF

Graduate of an accredited school of registered nursing; BSN preferred. Currently licensed as an RN through the Ohio Board of Nursing; must be able to obtain a license in the state of Michigan. Minimum 2 years of medical-surgical nursing experience; previous experience in the nursing home environment and/or hospice is preferred. Has working experience charting in an EMR system, Netsmart system preferred. Possesses a strong clinical background with knowledge of disease processes and pathways. Possesses clinical expertise related to critical thinking, assessment, problem-solving and technical skills and be able to multitask. Knowledge of Medicare hospice regulations and Joint Commission regulations preferred. Demonstrates the ability to seek, form and foster an effective, collaborative caregiving partnership with the nursing facility staff. Demonstrates awareness and sensitivity to the unique mission and cultural climate of each facility environment. Possesses strong verbal and written communication skills; demonstrates the ability to communicate effectively with patients, families, and team members regarding care issues. Has the ability to work independently and within an interdisciplinary team. Must be flexible and adaptable to change. Demonstrates strong organizational, time management, and prioritization skills. Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements. Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles. Meets state requirements for maintaining professional license. Must be CPR certified according to the American Heart Association CPR standards.
