Responsibilities:
-Coordinate patient care plans and discharge needs throughout hospitalization.
-Conduct comprehensive assessments to identify barriers to discharge and continuity of care.
-Collaborate with physicians, nurses, social workers, and interdisciplinary teams to develop effective care plans.
-Facilitate timely discharges and appropriate transitions to post-acute care settings.
-Educate patients and families regarding treatment plans, resources, and discharge instructions.
-Advocate for patients while ensuring quality, cost-effective care delivery.
-Maintain accurate and timely clinical documentation.
-Support compliance with hospital policies, regulatory requirements, and quality standards.
-Facility Type: Acute Care Hospital

 

Minimum Qualifications:
-Active California Registered Nurse (RN) License
-Minimum of 1+ years of recent Acute Care Hospital Case Management experience
-Recent experience performing:
-Care Coordination
-Discharge Planning
-Utilization of community and post-acute resources
-Acute Care Hospital Case Management experience is required
