Key Responsibilities

• Coordinate patient care throughout hospitalization from admission through discharge.
• Develop, implement, revise, and evaluate individualized case management plans.
• Collaborate with physicians, nurses, social workers, therapists, and multidisciplinary healthcare teams to optimize patient outcomes.
• Assess patients' clinical, psychosocial, financial, and discharge planning needs.
• Utilize Epic and Cerner electronic medical record systems for documentation, care coordination, and patient tracking.

Requirements

• Active California Registered Nurse (RN) License
• BSN Preferred
• Previous Case Management/Care Coordination experience preferred
• Recent Epic and Cerner experience within the last 5 years (required for Epic conversion project)
