As the Ridge Case Manager , you will collaborate with other members of the clinical team, to serve as a liaison for the treatment team, client, families and aftercare services.
Key Responsibilities:
- Provide comprehensive case management, including assessment, care planning, and ongoing support.
- Participate in the interdisciplinary treatment planning process and perform proactive discharge planning.
- Provide education to clients and their families regarding community resources, benefits, and coverage.
- Coordinate and facilitate meetings between the clients/families/residential care team. You will assist clients and families in making difficult decisions while honoring their preferences and values.
- Facilitate group sessions and support recovery-focused programming.
- Ability to successfully document all clinical contacts and documentation through Electronic Medical System.
- Support a safe, structured, and trauma-informed residential environment.
- Facilitate and provide timely documentation.
- Adhering to company policies and procedures regarding confidentiality and privacy.
- Other duties may be assigned.
Qualifications:
- Bachelor's degree in social work, psychology, healthcare administration, or a related field (preferred).
- Minimum 2 years of experience in a residential or therapeutic setting.
- Leadership skills that foster a culture of inclusivity and excellence.
- Excellent communication, leadership, and organizational skills.
- Strong crisis management and conflict resolution skills.
Salary Range
$28—$30 USD
