Key Responsibilities

• Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals aimed at progressing patients toward discharge within an established length of stay
• Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services
• Manage individual, concurrent, and group treatment sessions based on patient care needs, demonstrating flexibility in all aspects of caseload management
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines
• Participate in student programs as assigned, providing a positive learning environment with appropriate supervision, development of learning objectives, and completion of student evaluations
• Adhere to discipline-specific state practice acts and regulatory agency requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements

Required Qualifications

• Active Physical Therapist licensure in good standing in the state of Kentucky
• Completion of all state-mandated continuing education requirements
• Demonstrated competencies necessary to perform job functions in accordance with population-served criteria
• Commitment to patient and employee safety standards, including safe patient handling, infection control, and workplace hazard reporting

Preferred Experience / Skills

• Prior experience in an inpatient rehabilitation setting
• Experience with interdisciplinary team collaboration and discharge planning
• Involvement in professional organizations, continuing education, and community advocacy efforts
