As a Case Manager, you will be responsible for coordinating and providing support to individuals in need of social services. Your role will involve assessing client needs, developing treatment plans, and linking them to various resources and services. We are looking for someone who is compassionate, detail-oriented, and has excellent communication skills.Key Responsibilities:Conduct comprehensive assessments to identify client needs and develop appropriate treatment plansLink clients to resources and services such as housing, healthcare, education, and employmentMonitor client progress and provide ongoing support and guidanceAdvocate for clients and collaborate with other professionals, agencies, and organizationsMaintain accurate and up-to-date client records and reportsProvide crisis intervention and address any problems or issues that may ariseEducate clients and their families on community resources and support networksDevelop and facilitate support groups and educational workshopsQualifications:Bachelor's or Master's degree in social work, psychology, counseling, or related fieldMinimum of 2 years of experience in case management or related fieldKnowledge of social services and community resourcesExcellent communication, organization, and interpersonal skillsAbility to work independently and as part of a teamExperience working with diverse populations, such as low-income families, refugees, and individuals with disabilitiesProficient in Microsoft Office and database management systemsBilingual in English and Spanish is a plusBenefits:At Vupico, we value our employees and offer a competitive salary and benefits package to show our appreciation.
