Responsibilities Evaluate & Treat: Conduct initial and periodic assessments for speech, language, and swallowing disorders. Goal Setting: Develop and adjust measurable treatment plans based on individual patient needs and physician orders. Collaborate: Partner with the Rehabilitation Therapist and nursing staff on care transitions and discharge planning. Mentor: Provide clinical guidance to new staff, students, and volunteers. Qualifications Licensure: Current New York State SLP License and registrationNYS Office of the Professions. Required Experience: Minimum 1 year in a nursing home; rehabilitation setting;Patient Driven Payment Model (PDPM) experience required Preferred Experience: experience with pediatric patients Education: Master's Degree in Speech-Language Pathology from an accredited program. Physical Requirements Ability to move throughout the facility and stand/sit for extended periods. Capability to lift or position up to 30 lbs occasionally.
