We are currently looking for a certified Nurse Practitioner to join our expanding team at Primary Care Plus (PCPlus), a value-driven care delivery organization. Our goal is to guide our patients toward well-being through high-quality, cost-effective, and coordinated care that ensures excellent outcomes and satisfaction.

Key Responsibilities

• Deliver primary care services, including ordering and interpreting diagnostic tests and managing minor acute illnesses.
• Conduct thorough wellness assessments for members, either in-clinic or at their homes as necessary.
• Evaluate, diagnose, and manage chronic conditions within the patient population.
• Identify health deviations, initiate treatment, and monitor patients with common health issues within your practice scope.
• Prescribe medications according to established protocols and within your licensed authority.
• Provide personalized patient education and counseling.
• Engage in quality improvement initiatives and monitor health outcomes for patients.
• Ensure accurate coding and documentation for HEDIS, Quality STAR measures, and Risk Adjustment diagnoses as required.
• Participate in community outreach events, including health fairs and educational lectures.
• Collaborate with the marketing team to enhance practice visibility and expand our community reach.

Required Qualifications

• Completion of an accredited advanced practice Nurse Education Program.
• Certification as a Nurse Practitioner in Adult, Geriatric, or Family practice.
• Current licensure from the Louisiana Board of Nurse Examiners or the ability to obtain it before starting.
• CPR certification.
• Valid DEA license or the ability to secure one prior to employment.
• Clinical expertise to make informed recommendations regarding member assessments and management.
• Proficient in computer use and experienced with Electronic Medical Records.

Preferred Qualifications

• Experience in Gerontology, Internal Medicine, or Family Medicine.
• Background in managed care or ambulatory settings.
• Experience in urgent care.

The compensation for this specialty typically ranges from $109,500 to $164,000 . Total cash compensation includes base salary and bonuses, influenced by various factors such as local labor markets, education, and experience, with potential increases based on productivity and performance. We adhere to all applicable minimum wage laws.
