Occupational Therapist (OT) - SNF / LTC

Position Summary A Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist (OT) to join their growing team for a direct hire/permanent assignment. Responsibilities Provide direct patient care to assess medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician. Set realistic and achievable goals for patients, document and record patient conditions, and educate patients and families in appropriate occupational therapy methods. Provide skilled occupational therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate, and modify care to meet goals of occupational therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications Graduate from an accredited school. Hold a current state license as an Occupational Therapist or be in the proactive process of obtaining one. Current CPR certification.
