Recruiter Information
- Full Time direct Hire
- Schedule: Monday-Friday 8am-5pm
- Perform assessments to PACE Patients and help them to remain independent and live safely
- Position provides comprehensive healthcare services to elderly participants in the PACE program
- Responsible for assessment, treatment planning, and coordination of care for program participants
- Collaborate with an interdisciplinary team to ensure holistic care delivery
- Focus on preventive care and management of chronic conditions
- Maintain detailed documentation and participate in care planning meetings
- Support patients and families with compassionate, patient-centered care
- Licensed OT in CA, Valid CA Driver's license and reliable transportation.
