Your Responsibilities

• Implementing thorough patient evaluations, ordering relevant diagnostic assessments, and designing tailored care plans
• Diagnosing and managing diverse acute, chronic, and episodic health issues, either in partnership with physicians or independently (as permitted by your credentials)
• Providing education to patients and their families on wellness, prevention practices, and chronic disease management
• Prescribing medications and implementing treatments in accordance with your scope of practice
• Engaging in follow-up care and chronic disease management
• Collaborating with interdisciplinary teams to deliver seamless, comprehensive care
• Accurately documenting patient information in electronic medical records (EMR)

Education and Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Essential Certifications

• Active Nurse Practitioner licensure in Pennsylvania
