PRN Occupational Therapist OT Home Health 5 to 10 Visits Per Week

This position is contingent and paid on a per visit basis (5 to 10 visits per week on average) ; orientation and additional training will be paid at a separate hourly rate. Responsibilities Provide ongoing comprehensive assessments of the patient's needs Perform assessments within 24-48 hours of receiving the initial referral Establish a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Occupational Therapy Educate patients and caregivers Record on a timely basis all evaluation data, treatments, and patient's response to therapy intervention Also records quality assurance data Prepare an assessment of the patient's condition and periodic evaluations and summaries Supervise Occupational Therapy Assistants and paraprofessionals as per agency policy Participate with staff, patients, and physicians in discharge planning activities. Adhere to Federal, State and Accreditation requirements including Medicare and Medicaid regulations Minimum Qualifications Occupational Therapy License in good standing within the State of Michigan Preferred - Two (2) or more years of OT experience, specifically in a community/home health care setting Must have knowledge of Medicare/CMS guidelines Experience using an EMR Must have your own reliable transportation, valid MI drivers license, with proof of auto insurance and vehicle registration Benefits Mileage Reimbursement 401k with company match #J-18808-Ljbffr No Place Like Home Health LLC
