MDS Coordinator

Silvercrest of Urbana is seeking a dedicated and detail-oriented MDS Coordinator (RN or LPN) to join our clinical leadership team. As our MDS Coordinator, you'll play a vital role in coordinating resident assessments, maximizing reimbursement, supporting interdisciplinary care planning, and ensuring compliance with state and federal regulations. What You'll Do Coordinate and complete MDS assessments in accordance with CMS and Ohio regulations. Ensure timely and accurate completion of all required assessments and submissions. Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members. Participate in resident care plan meetings and help develop individualized plans of care. Monitor clinical documentation to support quality outcomes and reimbursement. Review Medicare and insurance documentation for accuracy and compliance. Assist with survey readiness and regulatory compliance initiatives. Analyze quality measures and work with leadership to improve resident outcomes. Educate and support staff regarding documentation requirements and MDS processes. Qualifications Current Ohio RN or LPN license in good standing. Previous MDS experience in a skilled nursing facility preferred. Working knowledge of RAI process, PDPM, Medicare, Medicaid, and CMS regulations. Strong assessment, organizational, and critical thinking skills. Excellent communication and collaboration abilities. Experience with electronic medical records preferred. Why Silvercrest of Urbana?
