Performing regular reassessments to monitor progress and adjust care plans as needed. Providing patient and family education on condition management, medication adherence, and preventive health measures. Collaborating physicians, therapists, social workers, and families to ensure coordinated, comprehensive care. Maintaining accurate clinical documentation in compliance with Ohio Board of Nursing regulations and agency policies. Supervising and supporting Certified Home Health Aides (CHHAs) and other care team members. We want to be transparent on pay! Please see below for all pay details.
