Case Manager

Community Health Alliance-Ohio

Community Health Alliance is seeking an experienced Case Manager (QBHS) to support children, adolescents and adults living with mental health conditions. You will coordinate care, teach daily living skills and foster recovery through individualized services in a community-based setting with home visits.
