Bernalillo County

Bernalillo County is hiring a Community Case Manager to provide case management and counseling across residential, community-based, and jail-based settings. You will assess needs, determine levels of care, and coordinate referrals to treatment and social services. The role involves working with external agencies, families, and justice system personnel to support client progress and ensure quality service delivery.
