We are seeking a dedicated Home Health Occupational Therapist to join our growing clinical team and help patients maximize independence, safety, and functional outcomes. Responsibilities / Essential FunctionsClinical Care & AssessmentPerform comprehensive occupational therapy evaluations and reassessments, documenting findings accurately and within required timeframesObtain and document pertinent medical, functional, and psychosocial history from patients, families, and/or caregiversConduct home safety and environmental assessments and implement interventions to reduce fall risk and improve safetyEvaluate patient performance in ADLs and iADLs and incorporate findings into treatment planningComplete OASIS assessments accurately and submit within 48 hours of the OASIS visitPlan of Care & TreatmentDevelop, implement, and revise individualized Occupational Therapy Plans of Care based on patient needs and physician ordersSubmit Plans of Care and recommendations to the physician for review, approval, and ongoing coordinationProvide skilled therapeutic interventions to improve functional independence, safety, strength, and cognitive performanceEstablish measurable OT goals based on patient function and rehabilitation potentialCommunication & CoordinationMaintain effective communication with patients and families regarding treatment goals, progress, and outcomesCommunicate with physicians verbally and/or in writing regarding patient status, treatment recommendations, and therapy effectivenessCollaborate with interdisciplinary team members to coordinate care and ensure continuity of services, including participation in case conferences as scheduled or triggered by clinical eventsSupervision & ComplianceDevelop and implement the Plan of Care when OT is the only skilled discipline orderedSupervise HHAs in accordance with federal and state regulations when OT is the primary disciplineParticipate in Clinical Record Reviews (CRR) in collaboration with leadership as requiredQuality Improvement & Professional DevelopmentParticipate in Performance Improvement (PI/QAPI) activities, committees, and initiatives as assignedAttend and actively participate in staff meetings, in-services, case conferences, and required trainingsMaintain current professional licensure and pursue continuing education to enhance clinical competence and patient outcomesSkills & AttributesStrong clinical skills in ADL/iADL training, functional mobility, safety interventions, and adaptive equipmentKnowledge of home health regulations and standards (CMS Conditions of Participation, CDPH, ACHC)Excellent communication and interpersonal skillsMinimum QualificationsCurrent, active California Occupational Therapist license in good standingMinimum of two (2) years of experience as an Occupational Therapist, in a home health settingSuccessful completion of a criminal background checkCurrent CPR/BLS certificationValid California driver's license, reliable personal vehicle, and current automobile insuranceProficiency with electronic medical record (EMR) systems and mobile documentation toolsWhy Join HealingHaven?Flexible scheduling with autonomy in the fieldExposure to operations, clinical leadership, and agency developmentSupportive leadership team focused on quality and complianceMileage Reimbursement
