River Grove Health and Rehab is seeking a compassionate and creative Affinity Coordinator to lead engaging, person-centered activities for residents in our Memory Care Unit. The ideal candidate has a background in activities coordination or social work , along with experience working with individuals living with Alzheimer's disease or other forms of dementia . In this role, you'll create meaningful experiences that promote cognitive stimulation, emotional well-being, and social connection while collaborating with our interdisciplinary team to foster a warm, supportive, and home-like environment where every resident feels valued, engaged, and empowered.
Responsibilities
Plan and coordinate a structured activity program designed specifically for dementia residents, incorporating daily routines, therapeutic recreation, and individualized engagement opportunities.
Facilitate group and one-on-one activities such as music, art, sensory experiences, exercise, reminiscing sessions, and life enrichment events that align with residents’ interests and abilities.
Adapt activities to meet the changing physical and cognitive abilities of each resident, ensuring all participants can engage at their comfort level.
Provide emotional and social support by building trusting relationships with residents, offering companionship, and encouraging participation and self-expression.
Collaborate with nursing and social services teams to support residents’ care plans, monitor changes in behavior or mood, and communicate updates effectively.
Maintain accurate documentation of resident participation, activity attendance, and behavioral observations as required.
Assist in creating a positive, home-like atmosphere that supports the emotional well-being and dignity of all residents in the Affinity Care Unit.
Contribute to special events and family involvement activities, promoting community engagement within the unit.
Qualifications
Background in activities coordination or social work is strongly preferred
Excellent communication, creativity, and interpersonal skills.
Compassionate, patient, and adaptable when working with individuals experiencing memory loss.
Ability to work collaboratively as part of an interdisciplinary care team.
