Job Description Summary:

\n

To competently interpret the patients history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. Based on problems identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Actively participates in departmental functions.\n

Responsibilities And Duties:

\n

\n
\n90% - Direct Patient Care – Evaluates and treats patients after physician referral.\n
\nAdministers portions of the assessment that is assigned by the Physical Therapist (PT) in which Physical Therapy Assistant (PTA) has demonstrated to be competent.\n
\nAssists in reassessment process, reporting patient performance to the PT. \n
\nAssists PT in implementation of the patients plan of care based on occupational performance frames of reference(s): bio-mechanical, rehabilitation, sensorimotor. \n
\nAttends multidisciplinary patient care meetings. \n
\nPlans and helps coordinate discharge plan with multidisciplinary team. \n
\nEducates patient and caregiver/family. \n
\nRecommends Medical Equipment required for discharge. \n
\nProvides information to Physicians, Nurses, Care Managers, Social Workers, and other team members.\n
\n
\n5% \n
\nDocuments evaluation performance directed and interpreted by the PT, progress towards goals, discharge plan/continuity of care for the multidisciplinary team, and other involved medical and insurance parties. \n
\nAll documentation to be co-signed by the supervising PT. \n
\n
\n5% - Indirect Department Functions.\n
\nDelegates assignments to Rehab Technicians. \n
\nRemains involved in department process improvements utilization review and quality assurance program. \n
\nEducates others about Physical Therapy. \n
\nTakes active role in orienting new staff members.\n
\nAssists in clinical instruction of PTA students. \n
\nEducates other disciplines and medical/hospital personnel on PT purpose, goals and treatment policies/procedures.\n
\nAssumes other duties delegated by management.\n
\n

Minimum Qualifications:

\n

Bachelor's Degree: Physical Therapy (Required)DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles, LIC - Licensed in Field - State of Ohio, PT1 - Physical Therapist License - American Board of Physical Therapy Specialties\n

Additional Job Description:

Licensed in the State of Ohio. Basic computer skills. Entry level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical Experience
