We're seeking a dedicated and passionate Physical Therapist to deliver top-quality home health care and share in our vision of advancing better living and care for aging adults.

What We're Looking For:
- Degree in Physical Therapy (Doctorate, Master's, or Bachelor's)
- 24 years of experience preferred
- Valid driver's license and auto insurance
- Passion for empowering seniors to live independently
Core Responsibilities Include:
- Evaluate and treat patients as ordered by a physician
- Develop and implement personalized plans of care
- Educate and support patients and caregivers
- Collaborate with interdisciplinary team members
- Supervise PTAs and aides per state regulations
- Maintain accurate, timely documentation
- Engage in ongoing education and team meetings
