Physical Therapist

Role Overview: This is a Physical Therapist position focused on delivering one-on-one care to older adults in private residences and senior living communities. The role involves performing evaluations, developing individualized treatment plans, and providing evidence-based therapy sessions to enhance mobility, independence, and overall quality of life. Key Responsibilities Offer personalized physical therapy, including assessments and treatment delivery, in various settings Document patient progress accurately within the EMR system, adhering to regulatory standards Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists Educate patients, caregivers, and staff on safety, function, and aging in place strategies Incorporate telehealth and virtual care to extend access and flexibility Qualifications & Skills Valid Physical Therapy license or eligibility in the practice state Degree from an accredited program Basic proficiency with computers and EMR systems Current CPR certification Ability to meet physical demands, including lifting, transferring, repositioning, and driving to patient locations Benefits & Opportunities: The role offers competitive pay, benefits such as health insurance, 401(k), continuing education, career advancement, and optional telehealth components.
