Contract | Days | 13-Week Assignment
Evaluate and treat patients in a skilled nursing facility setting to restore function and improve quality of life
Develop and implement individualized physical therapy treatment plans based on patient needs and goals
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
Educate patients and caregivers on therapeutic exercises and home programs to support continued recovery
Active Physical Therapist (PT) license in the state of Colorado
Graduate of an accredited Physical Therapy program
Prior experience working in a clinical or rehabilitative setting
Strong documentation and communication skills
Previous experience in a skilled nursing facility (SNF) environment
Familiarity with Medicare Part A and Part B documentation requirements
Experience with interdisciplinary team collaboration in a post-acute care setting
