We're looking for a compassionate, skilled LPN to join our Hospice team in a full-time, field-based role serving patients and families throughout the Cleveland / Northeast Ohio area.

Compensation

• $30.00-45.00 per hour
• Expected 40 hours per week
• Additional coverage opportunities available in Home Health and Palliative Care

What You'll Do

• Provide direct patient care to hospice patients in their homes in accordance with the established plan of care
• Monitor and document patient symptoms, comfort levels, and changes in condition
• Administer medications and treatments as directed by the RN Case Manager or physician
• Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals
• Provide education and emotional support to patients and families navigating end-of-life care
• Maintain accurate and timely documentation in the EMR within required timeframes
• Provide coverage in Home Health and Palliative Care settings as needed

Requirements

• Valid LPN license in good standing with relevant state licensing boards.
• Proven experience in hospice or palliative care settings is highly preferred.
• Familiarity with EMR/EHR systems such as HCHB
• Knowledge of medical terminology related to geriatrics, dementia care (including Alzheimer's), infectious disease care, and pain management.
• Experience working with individuals with developmental disabilities or in assisted living environments is a plus.
• Strong assessment skills combined with physiology knowledge to evaluate patient needs effectively.
• Excellent communication skills for triage situations and family education; basic life support (BLS) certification is necessary.
