Inpatient Care Manager | Discharge & Coordination Lead

Advocate Health is seeking a registered nurse case manager at Advocate Trinity Hospital in Chicago. You will conduct assessments, plan discharge, coordinate care across teams, and ensure timely transitions. This full-time role requires a BSN, 2 years of clinical nursing experience, and strong knowledge of utilization management and Medicare guidelines.
