Member Health Risk Assessor & Care Coordinator

CareSource is seeking a Member Health Assessor to build professional relationships with members and conduct comprehensive health risk assessments. You will verify eligibility and demographics, gather information from members, families, providers, and other stakeholders, and support care plan development.
