As a Hospice RN specializing in case management, you will play a critical role in delivering comprehensive care to patients in their homes. Your responsibilities will include:

• Providing hospice care and case management in a home setting
• Coordinating with interdisciplinary teams to ensure holistic patient care
• Utilizing McKesson/Netsmart EMR for documentation
• Traveling to rural locations to provide care
• Working Monday through Friday with a focus on patient-centered care
• Need 1 year experience in setting
