We are seeking a dedicated Nurse Case Manager to provide comprehensive health-tracking coordination, profile management, and case monitoring for a dedicated federal healthcare delivery program. This on-site position focuses on managing care pathways, coordinating with external healthcare providers, and ensuring total administrative data compliance, Monday through Friday, daytime operations. Working as part of a specialized medical readiness team under a Project Lead, the Nurse Case Manager will execute system-driven care tracking and prepare files for formal medical evaluation boards. Occasional company-paid out-of-state travel is required to support centralized mobilization reviews. Flexibility is required to support scheduled operational readiness events per year.
Requirements:Clinical Case Management & Coordination
- Care Monitoring: Coordinate comprehensive, non-clinical care pathways for an active, fluctuating caseload of organizational members.
- Healthcare Systems Management: Actively input, track, and manage member medical records within specialized federal health tracking databases and electronic case management systems.
- Profile Initiation: Review provider referrals and establish electronic care profiles within mandated, tight turnaround timelines.
- Medical Charting: Maintain absolute accuracy and timeliness in charting; document and upload all member encounters and external clinical notes within standard operating review windows.
- Evaluation Board Support: Audit files, reconcile hardcopy and electronic histories for completeness, and provide updated status summaries to formal review boards.
- Operational Readiness Events: Support periodic, large-scale health assessment and deployment readiness review events throughout the year, utilizing flexible shift adjustments to maintain standard workweek constraints.
Program Compliance & Quality Support
- Quality Execution: Perform daily clinical coordination in strict alignment with the program's Quality Control Plan (QCP) and performance standards.
- Administrative Collaboration: Work alongside health information technicians and project leadership to streamline daily on-site operations and data validation.
- Data Integrity: Maintain precise documentation and metrics to assist in the compilation of required monthly profile management status reports.
- Information Protection: Follow strict privacy rules, HIPAA guidelines, and federal data protection policies during all member tracking and record handling activities.
Position Qualifications & Requirements
- Education: Graduate of an accredited professional nursing program. Preferably a Bachelor of Science in Nursing (BSN) or higher.
- Licensure: Must hold an active, unrestricted license as a Registered Nurse (RN). A valid multi-state Nurse Licensure Compact (NLC) license with privileges to practice in Vermont is fully acceptable.
- Clinical Experience: A minimum of two (2) years of direct clinical nursing or case management experience.
- Certifications: An active Certified Case Manager (CCM) credential or equivalent professional designation is a plus.
- Domain Expertise: Recent experience (within the past 5 years) utilizing Department of Defense (DoD) or military medical readiness databases and tracking systems is highly desirable.
- Security & Citizenship: Must be a U.S. Citizen capable of passing a federal Tier 1 background check (NACI) to secure an organizational credential and authorized network system access.
- Must be located or willing to relocate to Burlington, VT.
