Healthcare Care Coordinator
$19 per hourOur client is seeking a highly motivated and empathetic individual to join our team as a Care Coordinator. In this critical role, you will be the first point of contact for patients, physicians, and referral sources, providing compassionate support and efficient, first-call resolution. - Create and complete accurate referrals and applications using our centralized Customer Relationship Management (CRM) platform, staying updated on any policy or procedural changes.
- Resolve patients' questions and concerns regarding their patient journey status with clarity and empathy.
- Enter detailed, accurate, and clear information into our company proprietary software while conversing with patients via telephone.
- Place outbound phone calls for patient follow-ups or confirmations to ensure timely progression of their accounts.
- Maintain high-quality service while providing an empathetic and supportive experience to the patient by confidently controlling the conversation and clearly educating the caller.
- Interact with patient referral sources, which may include healthcare providers, to efficiently process new applicants.
- Take ownership of patient accounts from initial contact through final approval or denial, ensuring a seamless experience.
