The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.Key Responsibilities:Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing settingProvide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and proactively monitoring conditions to prevent complications and reduce hospitalizationsDeliver timely clinical interventions in response to changes in patient status, ensuring high-quality, patient-centered care that improves outcomes and supports functional well-beingSupport value-based care initiatives by focusing on quality outcomes, reducing avoidable hospital readmissions, optimizing cost of care, and utilizing data to guide clinical decision-making and performance improvementCollaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care, and participate in care plan meetings and quality improvement effortsFacilitate smooth transitions of care across settings, ensuring continuity, reducing gaps in treatment, and supporting successful admissions and dischargesEducate residents and families on diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and long-term healthMaintain accurate, timely, and compliant documentation while adhering to all clinical, regulatory, and organizational standardsQualifications:Master's degree in Nursing from an accredited Nurse Practitioner programActive and unrestricted Advanced Practice Nurse (APN) license in respective stateCurrent DEA registration and prescriptive authority (or eligibility)Board certification as a Nurse Practitioner (e.g., FNP, AGNP)2 years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settingsStrong clinical assessment, diagnostic, and treatment planning skillsKnowledge of chronic disease management and geriatric care principlesAbility to deliver high-quality, value-based care focused on outcomes, cost-efficiency, and reduced hospitalizationsProficiency in evidence-based practice and clinical decision-makingAbility to work both independently and collaboratively within an interdisciplinary teamExcellent communication and interpersonal skills with residents, families, and staffStrong organizational, time management, and documentation skillsFamiliarity with electronic health records (EHR) systemsAdditional Benefits:Excellent earning potential with competitive base salary plus uncapped bonus opportunities or flexible fee-for-service optionsConsistent, reliable referral flow so you can focus on patient care, not building a caseloadComprehensive benefits package including Health, Vision, and Dental insuranceGenerous paid time offFull malpractice coverage including tail insurance, no out-of-pocket costsRetirement planning support with a 401(k) and company matchOngoing professional growth through CEU opportunities and paid licensure renewalsSupportive, clinician-focused leadership team that prioritizes your success and well-beingUp to a $3,000 referral bonus, get rewarded for bringing great clinicians to the team
