Nurse Practitioner (NP) / Physician Assistant (PA)
Location: Decatur/Springfield, IL
Schedule: Full-Time | Monday–Friday | 40 Hours/Week | No Weekends or Holidays
Compensation: $180,000 (Base + Incentives)

About the Role
Join a growing mobile geriatric care team providing primary care, wellness, and chronic disease management services within assisted living and memory care communities. This role offers significant autonomy, a collaborative work environment, and the opportunity to make a meaningful impact on patient care.

Key Details

• Patient volume: 16–20 patients per day
• Mobile practice model – typically serving one community per day
• Regular visits to assisted living and memory care facilities

Requirements

• Minimum 2 years of NP or PA experience
• Active Illinois license
• DEA certification
  ~ST
