Mobile Health Referrals Liaison - Tidewater Area

Life Center of Galax seeks a Mobile Assessor to expand referral networks and coordinate admissions for clients with substance use recovery goals. You will collaborate with hospital discharge planners, social workers and physicians, manage leads, educate stakeholders and travel within the community to develop sources.
